District IV Football Coaches Association 2024 HS Testing Combine Registration
By submitting this electronic form I hereby release the District IV Football Coaches Association, Liberty Arena, Circle W Sports, camp directors, and officials of any and all liabilities assocaited with the District IV Football Coaches Association High School Testing Combine for injuries or losses suffered by me, directly or indirectly, in training for, traveling to or from, and participating in the camp. If participant's number is not returned, I understand that there will be an extra $5.00 charge for replacement cost.
The cost of attending the District IV Football Combine is $10.00.
